TLC Alumni Golf Championship Playoff Tickets Giveaway Official Rules

1. NO PURCHASE NECESSARY. The Sweepstakes begins at 12:00:00 a.m. CT on July 14, 2008 ("Start Date") and ends at 11:59:59 p.m. CT on August 15, 2008 ("End Date"). Sweepstakes is open only to TLC Alumni who had their procedure at the TLC Laser Eye Centers located in St. Louis or Springfield, Missouri, who are legal residents of the United States, who are listed as the first referral source for a family member or friend who then has a complimentary LASIK consultation at the TLC Laser Eye Centers® in St. Louis, Winghaven or Springfield, Missouri (“TLC Participating Centers”) between the Start Date and End Date. This Sweepstakes is subject to federal, state, and local laws and regulations and is VOID WHERE PROHIBITED BY LAW.
2. METHODS OF ENTRY. For each friend or family member that lists an eligible TLC Alumni as their first referral source and that has a FREE consultation with a TLC Participating Centers between the Start Date and End Date, TLC will automatically enter that TLC Alumni into the drawing. There is a limit of one entry per individual referred but no limit on the total number of entries per TLC Alumni.
3. PRIZE. Sweepstakes prize is two (2) tickets to a single day at the Golf Championship Playoff at Bellerive Country Club in St. Louis, Missouri (“Prize”). Actual date of tickets during Golf Championship Playoffs at Bellerive Country Club is subject to TLC’s sole discretion. Limit two (2) tickets per validated winner. Approximate retail value: $300 per Championship Club ticket.
4. DRAWING. On or about August 20, 2008, TLC will select the name of potential winners in a random drawing of all valid entries received. Six (6) winners will be randomly selected from eligible participants entered by the End Date to win 2 single day Championship Club tickets. The drawing will be conducted at TLC's corporate offices located at 16305 Swingley Ridge Road, Suite 300, Chesterfield, MO 63017. All results of the selection are final and binding, subject to Paragraph 10 below. TLC will attempt to notify potential winners by phone within five (5) days of the drawing. Except where prohibited, winning is contingent upon signing and returning a publicity release (compliant with HIPAA, if applicable), prior to receipt of Prize. In the event the winner is not able to comply with the above requirements, the Prize may be forfeited and an alternate winner selected. The Prize cannot be redeemed for cash and may not be refunded, exchanged, substituted but may be transferred by the winner. All tax liabilities and other costs, including travel costs, incurred by the winner are the sole responsibility of the winner.
TLCVision is not responsible for telecommunications, network, electronic, technical or computer failures of any kind, including any that may affect your entry or the Sweepstakes. TLC is not responsible for lost, stolen, misdirected, mutilated, late or illegible entries. TLC shall have the absolute right to determine if an entry is in compliance with these Official Rules, without notice to the person attempting to enter.
5. ODDS OF WINNING. Odds of winning depend upon the number of eligible entries received.
6. RELEASE OF LIABILITY. By submitting a referral and being automatically entered in the Sweepstakes, each entrant warrants that he or she has read and agrees to abide by these Official Rules and that he or she is not prohibited from participating in the Sweepstakes. Each participant agrees to release, discharge, indemnify and hold harmless TLC and its parents, subsidiaries, affiliates, shareholders, employees, officers, directors, agents, representatives, advertising and promotion agencies and prize suppliers from any liability arising from or related to the Sweepstakes or acceptance of the Prize, including without limitation, personal injury, death, property damage, or claims based on publicity rights, defamation, or invasion of privacy.
7. CONDITIONS OF PARTICIPATION. Employees and agents, including their family (parents, children, siblings, spouse) and members of their household (whether or not related), of TLC and its subsidiaries, affiliated companies, parent companies, divisions, or of any related entity, including any consultants to TLC, are not eligible to participate in the Sweepstakes.
8. DELIVERY OF PRIZE. All tickets must be picked up by validated winner between August 27th – 29th, 2008 at the TLC Laser Eye Centers located in St. Louis or Springfield, Missouri where the TLC Alumni had their procedure. TLC is not responsible for telecommunications, network, electronic, technical or computer failures or delivery problems of any kind.
9. WINNERS LIST/OFFICIAL RULES. To obtain a list of the official winners (after the conclusion of the Sweepstakes on August 15, 2008), send a self-addressed envelope with the proper postage, subject to state law exclusions, affixed to: Winner, "TLC Alumni Golf Championship Playoff at Bellerive Country Club Tickets Giveaway," 16305 Swingley Ridge Road, Suite 300, Chesterfield, MO 63017. Please be certain to specify that you are requesting the "Official Winner List for the TLC Alumni Golf Championship Playoff at Bellerive Country Club Tickets Giveaway." To obtain a copy of these Official Rules, send a self-addressed envelope with the proper postage, subject to state law exclusions, affixed to: Official Rules, "TLC Alumni Golf Championship Playoff at Bellerive Country Club Tickets Giveaway," 16305 Swingley Ridge Road, Suite 300, Chesterfield, MO 63017.
10. GENERAL CONDITIONS. By entering the Sweepstakes, participants consent to TLC’s use of participant’s name, likeness, image and biographical data for advertising and promotional purposes without additional compensation, except where prohibited by law. BECAUSE TLC COLLECTS PERSONAL INFORMATION FROM PARTICIPANTS ENTERING THE SWEEPSTAKES, TLC ENCOURAGES ALL PARTICIPANTS TO REVIEW ITS PRIVACY POLICY, WHICH CAN BE REVIEWED AT WWW.TLCVISION.COM. TLC reserves the right to terminate the Sweepstakes or modify any rules as necessary for any reason or as required by applicable law. TLC reserves the right, at its sole discretion, to disqualify any person it believes to have intentionally violated these Official Rules or tampered with any element of the Sweepstakes without notice to the disqualified person. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of entrant and TLC in connection with the Sweepstakes, shall be governed by, and construed in accordance with the laws of the State of Missouri, without giving effect to any choice of law or conflict of laws rules or provisions (whether of the State of Missouri or any other jurisdiction), that would cause the application of the laws of any jurisdiction other than the State of Missouri. The venue of any legal action regarding these Official Rules and/or the Sweepstakes will be a court located in the Eastern District of the State of Missouri.
